Roughly 60% of the cases he saw were some variation of “an external data source changed in a way the specification didn’t anticipate.” The tool worked perfectly until the world shifted underneath it. The foundation was fine. The relationship between them was what broke. A tractor did not spontaneously change its engine calibration because John Deere updated a database somewhere; physical tools degraded predictably, through wear and corrosion and fatigue, and you could see the degradation coming.
